This was issued as a 3-inch CD single in America at the time the song was released, which I own. I can confirm that the 45 is an early fade of the LP version. My 45 has a printed run time of 3:52 and an actual run time of about 3:54.

To recreate the 45 from the LP, keep 0:00.0-3:55.1 of the LP version, with a 32-beat fade from 3:39.1-3:55.1.

Although I can't say with 100% certainty, I'm pretty sure that Bon Jovi's "Bad Medicine" was an early fade of the LP Version on the 45.
