Every case is different, but, working in the music business, I'd say that most of the times it's the record label dictating how long they want the song to be as a single and that it's hardly ever the artist. (Most artists don't even want their work edited.) In terms of the actual start time of the fade (if that's what you're asking), that's probably decided by the engineer (with rough guidelines by the label of what they want.)

Perhaps, no one answer either. Here goes...with all the 45 versions out there that are simply early fades of the LP versions, and there are MANY, who makes the decision where the fade occurs? Is it the recording engineer, the record label, the artist?
